Dr. Amin and our providers coordinate with other specialists involved in your care to do what is best for you and your overall health. Services include routine diabetic office visits, diabetes and nutrition education, labs and foot exams

Services include blood pressure screenings, diet and nutrition education, labs and medication management.

Services include evaluation for coronary artery disease and stroke risk, diet and exercise education, and medication management.
Asthma
Services include medication management and Asthma Action Plans.
Heart Disease
Services include medication management and coordination of care with cardiologists.
Osteoporosis
Services include education, medication management and referrals for bone density screenings.
Arthritis
Services include medication management, labs and care for acute flareups including musculoskeletal injections.
Thyroid Abnormalities
Services include medication management, labs, diet and nutrition education.
Obesity
Services include labs, risk factor education, diet and nutrition education, and exercise planning.
We provide individually customized health care for people experiencing frequent setbacks or healthcare challenges. We want to help you reduce hospitalizations and support your recovery through a holistic approach. Care coordination is offered through our office team that includes primary health care, transportation, social relationships, and community participation.
We have a care coordinator who may contact you proactively if we feel we can help you. If you feel the need to be contacted please let us know.
To help you with best dietary guidelines we have employed a nutritionist who is available with appointment for consultations on topics such as Diabetes and Obesity. If you wish to set up a consultation please notify us. As a team our providers and the dietitian can provide following services:
Assess your health needs and diet
Counsel on nutrition issues and healthy eating habits
Develop meal plans, taking both cost and preferences into account
Evaluate the effects of meal plans and change the plans as needed
Promote better nutrition by speaking to groups about diet, nutrition, and the relationship between good eating habits and preventing or managing specific diseases
Keep up with the latest nutritional science research
Write reports to document your progress
As part of our comprehensive healthcare services we have several Co-Management agreements in place to ensure continuity of care. We currently have a Co-Management agreement with Behavioral Health as well as Urgent Care services.
Co-Management agreement is in place with behavioral health provider Dr. Barbara Lowry. She is located at Salisbury Psychiatric Associates at 427 W Innes St Ste 1 Salisbury, NC 28144. Agreement is in place to provide an additional layer of care that allows for behavioral health concerns to be addressed effectively. Her office is open M-Th 8am-7pm, closed on Fridays and Weekends and can be reached at (704)637-5151
We also have a Co-Management agreement in place with Novant Health Urgent Care & Occupational Medicine located at 1904 Jake Alexander Blvd Ste 301 Salisbury, NC 28147. Agreement is in place to ensure continuity of care and to meet all of your Urgent care needs. They are open M-F 7:30am-7:30pm, Sat 8am-6pm and Sun 8am-4pm and can be reached at (704)638-1551.
We know that sometimes healthcare needs cannot always be planned for in Urgent situations. Our office offers direct provider communication not only during regular office hours but also after hours. Just call our regular office phone number at (704)636-5626 to connect to Dr. Amin or another provider to address your concerns. Direct contact with a provider after hours should only be used for urgent matters and not for routine health needs i.e. prescription refills, scheduling, billing, etc.
